What Is Advance Care Planning?

Advance Care Planning is a process that gives people the chance to think about, and write down, what is important to them in regard of their future care. It is a process of discussion between the individual, their partner, family or friends and, depending on circumstances at the time, those who provide care (for example, nurses, doctors, care home manager or social worker).

There may be times in everyone’s life when they think about the consequences of becoming seriously ill or disabled. If they were to lose the capacity to make decisions about their everyday life, what are the important things the people caring should know, and what are the individual’s wishes about what should happen?
